I, as parent/guardian of the above registrant(s), for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS THE CITY OF PLANT CITY, their officers, agents and/or employees, other participants, sponsoring agencies, sponsors and advertisers (all of which are referred herein as “Releasees”) WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE. I give permission to use my child’s name and photographic image for posting rosters and results on the City website, reporting results to the media, use on social media and use in promotional materials. I have signed the Parent Code of Ethics and I am aware that violation of this Code of Conduct may result in disciplinary action and/or dismissal from the program.
